Order / Quote Form

To obtain your Order/Quote Form simply click on the

button which is below each picture.

To add another order use the



button which is usually located in the top left hand corner of your browser to go back to where you were previous and then click the "order" button again.

Continue to add items into your order.

Your order/quote should look something like this

If you want to order more than one item simple change the number in the "Qty" field - Then press "Recalculate".
If you would like to delete an item simply put a "0" in the "Qty" field - Then press "Recalculate".

Once you are satisfied with your order proceed to "Place Order"

At this point you have 4 options:

OPTIONS:

1. Order Online - Fill out the form, place your credit card detail in the spaces provided and press SUBMIT.

2. Print / Fax - Fill out form, place your credit card details in the spaces provided, then PRINT out the form and FAX the form on (03) 9387 4829.

3. Print / Fax / Cheque - Fill out form, PRINT page, then send with CHEQUE to: Co-ordinates Therapy Services, PO Box 59, West Brunswick, VIC. 3055. Australia. Cheques made out to: Co-ordinates Therapy Services.

4. Quote - Complete form PRINT out and retain for your reference. Attach to purchase order.

Prices are in Australian Dollars including 10% GST. Overseas orders are GST exempt but have additional shipping charges minimum of $20.00 and will vary according to weight.
